Advisor Selection Form: You get this form from Gail and give it back to Gail. As soon as possible, students should form a committee, composed of three graduate faculties, one of whom will serve as the chair. The committee chair will be the major advisor throughout the degree work at Washington University. This committee can be changed over the course of their studies. Students should meet with their entire committee to plan their program. Note that you can fill it out whenever you feel like it after beginning grad school, but it must be on file before Apr/May of your first year.
As soon as possible, but not less than one week before defense date, give Gail the following information:
For your oral defense, you'll need to fill out a few forms. These *should* all be given to you in a folder from Gail. In that folder you should find:
These all need to be submitted to the Graduate School office and copies need to be given to Gail. I HIGHLY suggest making copies for your own keeping, as well!!
At some point during this time, you'll need to fill out the Title, Scope, and Procedure form (found on Gail's Forms website). The TSP form MUST be filed WELL before you intend to defend (at least 6 months!). You can, however, fill it out as soon as you run out of credits. The TSP form changes your "official" status to ABD (all but dissertation). This form is a written contract between the advisor committee and student stating the outline of the planed PhD thesis and can be changed whenever and as how many times as it is necessary but only with the agreement between student and their advisor committee. The chair of the department must also approve the program before it is sent to the Graduate School. This form also has to be filled out in case you apply for the Deans fellowship in your last year.
The Intent to Graduate form (yes, again!) must also be filled out within 6 months of defending. Most people fill it out on WebSTAC, but you can get the hard copy from Gail.
